Anyone who says they want to go outside is sent out in a suit to "clean" the camera lens - and always does, then dies. The helmet display shows a green, living world; the tape, the suit, and the screen are all part of why people clean.

S1E1Holston cleans and dies, establishing the ritual and the false display.
S1E10Juliette refuses to clean and survives, exposing the display as a fabrication.
S3E7The nature of "the outside kills you" gets more complicated: the surface attack on Lukas and Kennedy shows an active, policed kill layer (drones/automated guns), not only poison air. That reframes cleaning deaths, the Silo 17 bodies and Solo's "the poison came back" memory - the danger may be layered (suit/gas, dust/nanotech, and armed surface defense), and Juliette's survival may have been the Voice simply not expecting a cleaner to live.
